Proposal Approval Form

The Proposal Approval Form (PAF) accompanies the proposal or application as it is routed around the University for approvals before submission to a potential sponsor. The PAF is a three-page summary of information about the project. It is used for internal review and approval and data management. The PAF is not submitted to the proposed sponsor.

Important:
Instructions
  1. Click on the link to the "ePAF." You will see a login screen.
  2. Login using your uniqname and Kerberos password. You will see a list of your PAFs (if you have created any.)
  3. To create a new PAF, click on "New"
  4. Fill in pages 1-3 as necessary.
  5. If you want a subgrant distribution, fill in page 4. This page is optional.
  6. Save Changes
Printing
  1. If you click Print Preview while in Pages 1-4, the ePaf will first be saved before directing you to another page to download the form. "Any changes to the ePAF have been saved, to continue, please click the download link. Download".
  2. If you click Print Preview while in the ePAF listing, it will immediately download the ePAF file to your computer.
  3. Print this PDF version of the PAF. (You may also use this preview button at any time to see what the PAF will look like when printed.
  4. Route the printed version of the PAF for signatures, per usual. ( Be sure it arrives at DRDA within the deadlines spelled out in our service policy.)
  5. Mac OS X - Preview - Preview WILL NOT show the completed form, you need to use Adobe Acrobat (Reader).

Clone an ePAF
Only the owner of an ePAF can clone it. To use an existing ePAF as a template for a new ePAF - click on "Clone". A new ePAF is created from the original with the word CLONE: preceding the original title.

Delete an existing ePAF
Only the owner of an ePAF can delete it. The owner will also be asked to confirm the delete request before the ePAF is deleted.

Change Ownership of an ePAF
Only the owner of an ePAF can assign the file to a new owner, thus granting that person the ability to clone and delete the file.

Manage your profile (and workgroup)
The "Profile" button will allow you to grant access to your ePAF files to other people at the university. This affects all your files. Access is granted to all ePAFs you own, it is not granted on a file-by-file basis. Do not remove yourself from your own profile, you will lose access to your own files!
